There is no consensus as to what mileage they need to be replaced because there have been failures in engines as low as 17k miles in CPO cars. How it was maintained seems to have zero influence on when the bearings might grenade.

Those who believe there is an underlying clearance/engineering/assembly problem inherent to these S65 would generally suggest that you replace them as soon as you get the car. Even if for no other reason than to have an insurance policy against future failure.
