Helmut Marko lost an eye because of a projectile on track.
"At the French Grand Prix on July 2, 1972, at the Clermont-Ferrand circuit, Helmut Marko was forced to end his racing career. On lap eight, while closely following Emerson Fittipaldi, he was struck in the face by a stone kicked up by Fittipaldi’s car as it cut a corner. The rock pierced his helmet visor and caused irreparable damage to his left eye. Despite the injury, Marko managed to stop the car before losing consciousness. He was assisted by Vic Elford and taken to the medical center, but the initial treatment worsened the condition. He was only taken to a hospital later that evening. Since then, he has worn a prosthetic eye."

The halo is so damn 'fugly (fugly is exceedingly ugly) but one of the best F1 safety features ever.
