engine fan runs at high speed and P0128

Once the weather started getting warmer my engine fan began running at high speed and would continue throughout my drive no matter how long the drive was. Sometimes the fan would stop when the engine temperature reached somewhere near 160°F.

This weekend I replaced the thermostat and the thermostat temperature sensor, the water pump and the upper and lower hoses. The problem still exists after doing all of that. Before doing the cooling system overhaul I had already cleared the codes about a week ago and codes are present.

Any advice ideas appreciated.
