Mains wear, same as rods just at a slower rate since the crank doesn't have the same forces applied to it as a rod does (up/down). They suffer from the same clearance and tolerance stacking issue that the rods do. Therefore, same as the rod bearing issue there is no way to determine when they'll fail or specify some mileage at which they should be replaced.

There's no silver bullet. The best you can do is maintaining a frequent oil service interval.
