I think there is some sizing advice on the necks gen web site, but basically the medium fits for most. Only reason to go larger might be for very strong neck sizes; I wear 16.5 shirt collars and it fits fine.
All the Hans devices require two shoulder belts; I used these with the Schroth quick-fit pro harness, which is a dot-approved 4-point belt system that does not need a harness bar.
