And for the door skins, you're saying the skins don't appear to be an option, so the entire door sans inner door cards would need to be replaced? I know the driver's lock is fubar, too.

Anything other than #1 will require some body filler, but it will be very minimal to cover welds. #1 should not require filler. #4 would be the other option that would require some small amount of filler.
Yeah, you would replace the door shell. all the components, cards, glass, etc. would be transferred to the new shell.
