Form for M school?

I remember a couple of months ago I came across a thread that had a link to the form you need to fill out for the free one day M school. I tried searching but can't find it. Does anyone know where I can find it? Thanks in advance.

Choose the M School you want and the date and you will get to a form. Alternatively, you can call the 1-888 number on the website and they will send you a form along with a nice brochure and DVD about the Performance Driving School

If you bought your car brand new from a dealership between Sept 1, 2008 and December 31. 2008, you qualify for the free M School.
Otherwise, you have to pay for it. I did it (check out the latest issue of the Roundel with my buddy's recount of our experience ) and it was a blast.
