Grounding Problem?

I am trying to use an AV/charging cable to connect my iphone and multimedia interface kit. When the USB charging cable is not plugged into a 12V receptacle, the audio and video are fine. But if the USB charger is plugged in, I get a hissing noise on the audio. I've tried 2 different cable and 2 different lighter sockets, but I get the same noise.

Is this a grounding issue or is it a common problem with the iphone 3G?
