BMW usually sells fitted blocks, or better known as blocks with pistons. I suspect the same is true for an S65, but maybe someone else can confirm that?

No, never trust the prices shown in realoem as they are always wrong (for whatever reason). Take that part number and put it into BMW Morristown's online parts site. The BMWNA MSRP on that part is $6395, and you can but it from them for $4713.

Please, be sensitive, it's "little" block. It's the engine block, pistons and crankshaft. Basically, the lower half of the motor.

shortblock is the engine without the heads. It includes the pistons already fitted.
