Distinction without a difference. It doesn't matter to the customer whether the cause is a design error, a manufacturing error, or a supplier screwup. It's still a problem that the customer won't want to put up with.
And bugs don't exist in modern engineering? I guess it depends on what you consider a bug. I would consider all of the above a bug, and all of them definitely still exist.
