We got them from Amarr through Costco:

They have some standard wood designs but they will build anything you want. We provided them sketches from our architect and then they built them exactly how we wanted them. They are mahogany with seeded glass. Doors, hardware, installation, essentially everything ran about $6k per door. It's probably about 25% to 30% less if you go with a standard design.
