A rod cap not torqued, bearing too tight, wrist pin retainer clip not installed, etc. Parts subjected to reciprocating forces in an engine generally don't stand up long if they are defective, improperly torqued or improperly assembled they show up quickly!
Not necessarily,you can have a small crack that develops over time and result in a engine failure down the road in 500 miles or 20k miles. Like an unbalanced crankshaft...
